Volunteer Application

(NOTE: Please read the requirements on the Volunteer page before filling out an application.)

If you are interested in volunteering at Morris Hospital, please complete the application below and hit submit to send it to our Manager of Volunteer Services. Or, you can print out a paper application form, complete it at home and mail to: Morris Hospital & Healthcare Centers, Volunteer Services, 150 W. High Street, Morris, IL 60450.

All volunteer applicants must submit to a background check. After your volunteer application is reviewed, background screening instructions will be emailed to you from the Volunteer Manager.

Please note that we do not offer volunteer opportunities to fulfill court mandated community service requirements.
