Channahon Healthcare Center of Morris Hospital offers health services for patients of all ages, from infants to the elderly. Our primary care and specialty providers see patients for routine checkups, well-woman care, well-baby and child care, school and sports physicals, immunizations, behavioral health, and chronic conditions and illness. PROVIDERS:

PETER ANALYTIS, M.D., NEUROLOGY, SLEEP MEDICINE

Dr. Peter Analytis is a neurologist with a special interest in sleep medicine including treatment of sleep apnea, insomnia, narcolepsy, sleep walking, and other sleep disorders. When a sleep disorder is diagnosed, treatments can range from non-invasive breathing machines, oral appliance devices, medication, lifestyle change recommendations and in some cases surgery.

In addition to sleep medicine, Dr. Analytis specializes in the diagnosis and treatment of disorders that affect the brain, spinal cord, and nerves, including stroke, headaches, seizures, speech and language disorders, infections of the brain and peripheral nervous system, movement disorders such as Parkinson’s disease, sleep disorders, dementia disorders such as Alzheimer’s, and multiple sclerosis.

Dr. Analytis graduated from medical school at the University of Illinois at Chicago. Dr. Analytis has served on Morris Hospital’s medical staff since 1989.

SARAH BOJAK, F.N.P., FAMILY NURSE PRACTITIONER

Sarah Bojak, FNP-BC, Channahon Healthcare CenterAs a family nurse practitioner, Sarah Bojak is specially trained to care for patients of all ages, from babies to seniors, including preventive care, routine check-ups, health risk assessments, immunizations, screening tests, behavioral health, and personalized counseling on maintaining a healthy lifestyle. She holds a Family Nurse Practitioner certificate, allowing her to order and interpret diagnostic tests, diagnose disease, provide appropriate treatments and prescribe medications.

Sarah received her master’s degree in nursing from Olivet Nazarene University in Bourbonnais, Ill. and her bachelor’s degree in nursing from the University of St. Francis in Joliet, Ill. She is certified by the American Association of Nurse Practitioners and has completed completed the University of California Irvine Train New Trainers Primary Care Psychiatry Fellowship.

She is committed to providing compassionate and comprehensive care, encouraging each patient to actively participate in their care. She believes it is essential to build relationships with every patient through trust and communication.

Sarah has worked as a registered nurse since 2001 and has experience as a family nurse practitioner in an immediate care setting.

NUZHAT CHALISA, M.D., ENDOCRINOLOGY

Dr. Nuzhat Chalisa, Channahon Healthcare CenterMedical conditions associated with the glands and hormones often require the expertise of a physician who specializes in the endocrine system. Dr. Nuzhat Chalisa  with the Morris Hospital Endocrinology Specialists provides diagnosis and treatment of diabetes, thyroid disorders, osteoporosis, obesity and pituitary problems.

Dr. Chalisa believes in the principle of “Act on Diabetes Now,” which provides a multi-systemic approach to the promotion of diabetes education as a core component of diabetes care. She is committed to providing excellence in the care of diabetes and other endocrine disorders.

Dr. Chalisa completed her residency at Loyola University Medical Center and a fellowship in endocrinology at Rosalind & Franklin University of Health Sciences/Chicago Medical School. She is board certified in internal medicine and endocrinology.

A resident of Naperville, Dr. Chalisa is married and has two children. She enjoys traveling and interior decorating and is fluent in English, Urdu, Hindi, and Gujrati.

MARY CONNOLLY, M.D., FAMILY MEDICINE

Dr. Mary Connolly, Channahon Healthcare CenterAs a family medicine physician, Dr. Mary Connolly is specially trained to care for patients of all ages, from babies to seniors, including preventive care, routine check-ups, health risk assessments, immunizations, screening tests, behavioral health, and personalized counseling on maintaining a healthy lifestyle. She is committed to providing high quality care for her patients, along with clinical excellence and compassion.

Dr. Connolly received her medical degree at The Rosalind Franklin School of Medicine and Science/The Chicago Medical School and completed a residency at MacNeal Family Medicine Residency Program in Berwyn, IL.

When she’s not taking care of patients, Dr. Connolly enjoys spending time with her friends and family (specifically, spoiling her nieces and nephews!). Dr. Connolly has a personal interest in becoming more proficient in Spanish.

Jennifer Greggain  is an endocrinology nurse practitioner serving under Dr. Nuzhat Chalisa with the Morris Hospital Endocrinology Specialists. She is qualified to diagnosis and treat endocrine disorders, including diabetes, thyroid disorders, polycystic ovarian syndrome, osteoporosis, obesity and pituitary problems. She encourages patients to be actively involved in their healthcare decisions.

Jennifer received her Master’s of Nursing – Family Nurse Practitioner degree from University of St. Francis in Joliet. She enjoys spending time with family, reading, gardening and staying physically active.

STEFANIE HENLINE, N.P., NEUROLOGY

Stefanie Henline is a nurse practitioner specializing in neurology serving under Dr. Isaac Mezo. BELAL SAID, M.D., RHEUMATOLOGY

As a board-certified rheumatologist, Dr. Belal Said specializes in the diagnosis, management and treatment of musculoskeletal and rheumatic disease, including bursitis, gout/pseudogout, lupus and related diseases, osteoarthritis, psoriatic arthritis and rheumatoid arthritis. Many of these conditions are characterized by joint pain and inflammation, as well as muscle soreness and stiffness. Dr. Said is committed to providing compassionate care to patients 18 and older.

Dr. Said is a graduate of Jordan University of Science and Technology, School of Medicine, in Irbid, Jordan. He completed an internal medicine residency at St. Vincent’s Medical Center in Bridgeport, Conn., and a rheumatology fellowship at Columbia University Medical Center. Dr. Said is board certified in internal medicine and rheumatology. He is also a member of the American College of Rheumatology.

For your convenience, we suggest that you download and complete our Healthcare Centers – General Patient Registration Form before coming in for your office visit.

Please arrive 10-15 minutes prior to your appointment time to complete registration and a health questionnaire.

When you come for your appointments, please remember to bring the following:

  • Driver’s License or a valid ID
  • Insurance information
  • Referral Letter (if required)
  • Reports, X-rays, MRIs, CT scans, etc. and any other relevant information
  • List of medications (if any)

We recognize that your time is valuable, and we make every effort to stay on schedule. Occasionally emergencies or patients require a little more time and may cause delays beyond our control. We apologize if we keep you waiting.
