MEET THE PROVIDERS:
Dr. John Bolden, M.D., Infectious Disease
As a fellowship-trained infectious disease physician, Dr. Bolden specializes in conditions caused by pathogenic agents, such as bacteria, viruses, fungi or parasites, which may or may not be contagious. He has specific interest in antibiotic resistance, skin and soft tissue infections, HIV infectious, Clostridium Difficile Colitis and MRSA.
Dr. Bolden attended the University of Illinois-Chicago, where he acquired his medical degree and his master’s degree in cell biology. He completed his residency at Mercy Hospital & Medical Center in Chicago and a fellowship in infectious disease at Mount Sinai Hospital in Chicago and Rosalind Franklin University of Medicine in North Chicago.
He is the chairman of infection control at Morris Hospital & Healthcare Centers. In 2011, Dr. Bolden was recognized as one of the Top 25 African American Physicians in the Chicago area in Black Health Magazine.

Bradley Lawton, M.D., Family Medicine
As a board-certified family medicine physician, Dr. Bradley Lawton provides primary care for children ages 2 and older, adolescents, adults and seniors.
In addition to diagnosing and treating illness and injuries, Dr. Lawton is an excellent resource for preventive care, routine checkups, physicals, acute and chronic illness, behavioral health, immunizations, screening tests and personalized counseling on maintaining a healthy lifestyle.
Dr. Lawton completed a three-year residency in family medicine at LaGrange Family Practice Residency Program in LaGrange, Ill. He received his medical degree from the University of Illinois at Rockford. He is board certified by the American Board of Family Medicine and is a member of the American Academy of Family Physicians.

As an internal medicine physician, Dr. Peter Roumeliotis provides primary care services for adult patients. He has an interest in geriatrics and behavioral health.
He is a graduate of the University of Patras in Greece and completed an internship and residency at Mercy Hospital & Medical Center in Chicago. Dr. Roumeliotis has served on the Morris Hospital Medical Staff since 1991 and currently serves on the Morris Hospital & Healthcare Centers’ Board of Directors.

As a family nurse practitioner, Diana Olsen is specially trained to care for patients of all ages, including routine check-ups, preventive medicine, immunizations and physicals, illness and injuries, nutrition and lifestyle counseling, behavioral health, health risk assessments, and personalized counseling on maintaining a healthy lifestyle. She also has a clinical interest in treating diabetes.
Diana received her master’s degree in nursing from Lewis University in Romeoville and is ANCC board certified. She has a bachelor’s degree in nursing from Lewis University and a bachelor’s degree in biology from Northern Illinois University in DeKalb.

Olga Peplos, M.D., Internal Medicine

Dr. Peplos is a graduate of the University of Athens Medical School in Greece and completed an internal medicine residency at Illinois Masonic Hospital in Chicago. She completed an internship at St. Joseph Hospital in Chicago. Dr. Peplos has served on the Morris Hospital Medical Staff since 2001.

Diane Zuelke, F.N.P.-B.C., Family Medicine
As a family nurse practitioner, Diane Zuelke is specially trained to care for patients of all ages, from babies to seniors, including preventive care, routine check-ups, health risk assessments, immunizations, screening tests, behavioral health, and personalized counseling on maintaining a healthy lifestyle.
Her specialized training allows her to order and interpret diagnostic tests, diagnose disease, provide appropriate treatments, prescribe medications and offer guidance with behavioral health.
Diane acquired her bachelor’s and master’s degrees in nursing from the University of St. Francis in Joliet. She is a member of the Illinois Society for Advanced Practice Nursing and has completed the University of California Irvine Train New Trainers Primary Care Psychiatry Fellowship.
Diane strives to make sure every patient is involved in their own health care. She is committed to caring for and assisting all patients to achieve their health care goals.

How do I schedule an appointment?

How do I prepare for my appointment?
For your convenience, we suggest that you download and complete our Healthcare Centers – General Patient Registration Form before coming in for your office visit.
Please arrive 10-15 minutes prior to your appointment time to complete registration and a health questionnaire.
When you come for your appointments, please remember to bring the following:
- Driver’s License or a valid ID
- Insurance information
- Referral Letter (if required)
- Medical records from previous hospitals and providers including lab results, X-rays, MRIs, CT scans, etc., along with any other relevant information about your medical history
- List of medications (if any)

How do I cancel my appointment?
If you need to cancel an appointment, please call the office during business hours at least 1 day prior to your scheduled appointment so that we can offer your appointment time to patients on our waiting list.
Can a friend or relative bring a child under the age of 18 to their exam?
A parent/legal guardian must be present for well child exams.
For acute/sick visits, a parent/legal guardian must be present unless the parent/legal guardian has completed the Just In Case form or called the office in advance to make arrangements.

What is Prescription for Fitness?
Morris Hospital & Healthcare Centers and the Morris Hospital YMCA have teamed up to improve the health and wellness of area residents by offering a Prescription for Fitness program. Through this innovative initiative, patients will receive a FREE 30-day membership to the Morris Hospital YMCA when they are referred by a Morris Hospital provider.
The Prescription for Fitness membership includes a free fitness orientation. If you choose to join the Morris Hospital YMCA at the conclusion of your FREE 30 days, your joiner’s fee will be waived if you have visited the Y at least four times during your first 30 days.
